Some common problems to consider are as follows:
- The organisation has silos of excellence. Consider how these groups might integrate.
- Employees decision-making scope is too narrow or too broad.
- Individuals are incentivised to do the wrong things.
- Reporting relationships lead to compartmentalisation, or diffusion of accountability.
